Theorists and scholars alike have stated that the core component of the nursing profession is caring. Background Edith Louisa Cavell was born on December 4th, 1865, in Swardeston parish in the county of Norfolk in Eastern England. She was raised in a household comprised of strict Anglican beliefs enforced by her father, Reverend Frederick Cavell. It has been written that no books were allowed in the house except for the bible. Her devout religious faith would prove to be the guiding force behind her charity during her life. She began to train as a nurse in 1900 at the age of twenty at the London Hospital. Seven years later, she was recruited to become the matron of Berkendael Medical Institute in Brussels, Belgium. Not impressed with the current state of nursing in Belgium at the time, she sought to improve standards and regulate certain elements of practice by becoming an influential nurse educator. After the eruption of the First World War in 1914, Cavell vacated her again-home of England and returned to Belgium to resume her position as matron of the Berkendael Medical Institute, which had been converted by the Red Cross into a military hospital allowing the treatment of both German and Allied soldiers (Duffy, 2011). Despite Belgium’s declared neutrality, the country was promptly invaded and occupied by strict German forces. Cavell, knowing the inherent dangers of war, retained her post and continued to treat the sick and wounded. Knowing that many British soldiers were now trapped in German-occupied Belgium, her efforts were soon directed at assisting surrounded these British soldier’s in their return to England. Cavell was subsequently responsible for the safe removal of over 200 Allied soldiers from Belgium between 1914 and 1915. She provided shelter in safe houses, as well as false identification papers and guides out of the country. Unfortunately, she came under suspicion by the German military. This was not helped by her outspoken views on the perceived injustice of the German occupation (Tejvan, 2010). Cavell was apprehended by German authorities and eventually succumbed to interrogations. She was charged with treason and sentenced to death. Worldwide condemnation of the verdict (and the fact that she treated German and Allied soldiers indiscriminately) did little to detour the German military’s decision. Wearing a nursing uniform, Edith Cavell was executed by firing squad on the morning of Oct. 2, 1915. Global outrage ensued shortly thereafter. American and British mourners were particularly sensitive towards Cavell’s unjust execution; it ignited anti-German sentiment from both Americans and the British, serving as the catalyst for worldwide press coverage sympathetic towards the United States and Britain’s forthcoming war effort (Fee Roth, 2010, pp. 1865-1866). Followership Behaviour The concepts of leadership and followership are deeply intertwined (Grayson Speckhart). Edith Cavell’s traits as a follower allowed her to be an effective leader. The term follower is open to subjective interpretation, yet one particularly helpful definition is â€Å"[an individual] that follows the teachings and/or opinions of another† (Merriam-Webster, 2011). Grossman and Valiga have further expanded on the word ‘follower’ and have coined the term â€Å"effective follower† (2009, p. 41). In contrast to the above definition, the effective follower â€Å"functions independently, thinks critically about ideas that are proposed or directions that are suggested, and [is] actively involved† (Grossman Valiga, 2009, p. 1). They further suggest that effective followers have six common characteristics also possessed by effective leaders: assertiveness, determination, courage, an ability to act as a change agent, openness to new ideas and willingness to challenge ideas, and a willingness to serve (2009, p. 44). Edith Cavell displayed all of the above traits at one point or another during her lifetime which suggests she was an effective follower, and therefore, an effective leader. Edith Cavell was assertive, determined, and courageous. In defiance of the Red Cross’s code of non-military involvement (2011) as well as German military code of conduct, she continued to smuggle British men out of occupied Belgium despite knowing in full that a sentence of death by court martial was the penalty. Upon capture and interrogation by the German military, she confessed in full to her alleged crimes of treason, a testament to not only her courageous nature but her rigid abidance to her values. She continued to serve her country despite knowing it could possibly end in her demise. On a less dramatic note, she was a proponent of increasing the value placed on not only nurses but women in general. Doing this during a time when women’s issues were considered less important than men’s required ample courage. This is also consistent with the following point. Edith Cavell demonstrated effective followership behaviour (thus effective leadership behaviour) by being open to new ideas, possessing a willingness to challenge ideas, and acting as a change agent (Grossman Valiga, p. 44). Her willingness to challenge ideas was demonstrated by her public protest of the German occupation of Belgium and distain of German treatment of Allied soldiers. She acted as a change agent by inventing ways to smuggle Allied soldiers out of the country. Cavell also demonstrated these three behaviours during her stint as matron for the Berkendael Medical Institute. As described previously, she was unimpressed with what were current nursing standards in Brussels. For example, she instituted the practice of follow-up home visits for patients following discharge (Ryder, 1975). Cavell hypothesized that this would prevent readmissions, which indeed it did. It can also be said that Cavell was a foreword thinker on her views on gender. She was once quoted as saying, â€Å"The old idea that it is a disgrace for women to work is still held in Belgium and women of good birth and education still they think lose [status] by earning their own living†, suggesting that even in the early portion of the 20th century, Edith Cavell believed the notion of women being excluded from the workforce was antiquated. Lastly, Cavell possessed a â€Å"willingness to serve† (Grossman Valiga, 2009, p. 4). Grossman and Valiga suggest that loyalty is one trait commonly held by effective followers (2009, p. 44-45). Cavell was loyal and ‘served’ in two ways. First, she was extremely religious. As mentioned prior, she was raised in a strict Anglican household under the supervision of her pastor father, and thus was loyal to the teachings of Christianity. There exist two key elements inherently rooted in any religion: followership and caring. If one abides by the definition of followership stated prior then individuals practicing a religion, as was Edith Cavell, are followers. One of the core messages common to all biblical scripture, regardless of the chosen religion, is one of caring for other people and creating nurturing environments. Cavell’s selflessness was ultimately spurred on by her unwavering commitment and loyalty to the teachings of her religion. To demonstrate her faith, it has been said that during the two weeks she spent in solitary confinement prior to her execution the only books she requested were the Holy Bible and The Imitation of Christ, a medieval volume of meditations that emphasizes self-abnegation and suffering (Shaddox, 1999, p. ). Second, she had a willingness to serve her country. In addition to being a devout Christian, Cavell was a dedicated patriot. She possessed a love and devotion for Great Britain rivalled by few. This was demonstrated by her loyalty to her fatherland even in the face of formidable danger. It is interesting to note that this sense of patriotism was seemingly abandoned during her final hours of imprisonment by the German military, in which she uttered the famous quote mentioned at the beginning of this essay. Implications for Nursing The life of Edith Cavell impacted the nursing profession in a number of ways. On a superficial level, multiple hospitals and schools have been named in her honour (see Appendix). She was also in influential in the early creation of professional nursing journals. During her stint as matron in Brussels, she was credited with the launch of L’infirmiere, which documented proper nursing standards and practices (Tejvan, 2010). It can be said, however, that her greatest contribution may not have come in life, but in death. While execution by firing squad lays in the extreme, she continues to serve as a symbol to nurses and non-nurse’s alike as to the level of personal sacrifice and emotional dedication many nurses put into the profession every day. This was presumably never her original intention, but through martyrdom Edith Cavell will forever live as an inspiration to all nurses in the annals of nursing history. Conclusion Edith Cavell is an important figure in nursing history. Her ability to lead was preceded by her ability to demonstrate effective followership skills. Her patriotism and devout faith, both of which are rooted in the concept of followership, provided the foundation for many of her life’s endeavours. Ultimately, though, it was her loyalty to her fellow person which proved to be the true motivation behind her actions. Case 1-1 Ribbons and Bows Essay ?INTRODUCTION Carmen Diaz, with a ten thousand dollar loan from two of her cousins, and one thousand dollars that she invested in equity, was able to open a specialty store called Ribbons an’ Bows, Inc, which was located in Coconut Grove, Miami, Florida. Four months after opening the business, Carmen’s relatives requested a financial report, which was part of the original business arrangement. Within a short time, Carmen had expanded the business, purchased small equipment, paid wages to a part-time employee, and by all outward indicators, had been running a flourishing business. As Carmen gathered the necessary information to prepare financial statements, she started to scrutinize the transactions and was somewhat perplexed. Carmen realized that numbers were not adding up as she had anticipated. Additionally, it became questionable whether the business was operating successfully, at least when accounting principles were appropriately applied. Statement of the Problem It was fortuitous for Carmen Diaz to have family who were supportive of her small business endeavor, thereby providing start-up funds and legal advice. Carmen’s endeavor can be recognized as a â€Å"microbusiness† operation; â€Å"microcredit†, microenterprise† and â€Å"microfinance† are terms that have been associated with microbusinesses (Datar, 2009). Clearly, Carmen would have benefitted from some professional entrepreneurial guidance that is widely available at no cost in the United States. Indeed, her uncle practiced law, but it was not clear if business law (finance/ accounting) was within his forte. It was noticeable that Carmen was unskilled in accounting principles; she had apparently prepared a business plan but was not adept in all accounting proficiencies, for example, budgeting and long term planning. There was no evidence to indicate that Carmen had planned for the replacement of essential equipment, such as the computer and essential accounting management software, beyond the basic business tracking tools. She was not in tuned with important details of running a business. Altogether, Carmen was operating under a misapprehension about managing a business; one significant issue was that she was not paying herself a salary, and she had obviously not planned for such necessary compensation. In addition, Carmen had not make provisions for repayment of the loan from her family members. The major issue however, was that Carmen had spent more cash on the sewing machine, rent, employee wages and inventory, when compared with starting cash and the revenue that the business had generated (Appendix I: Income Statement). Carmen Diaz had encountered what has been described as â€Å"one of the most severe problems in microenterprise cost management† and that is the management of inventory (Datar, 2009). DESCRIPTION OF ALTERNATIVE SOLUTIONS According to Datar (2009), â€Å"access to financial capital isnt enough for many of the most vulnerable clients. Microbusiness owners often lack the knowledge and skills necessary to make effective use of financial capital. While it was necessary for Carmen to pay rent, wages and purchase small equipment to develop her business, she needed to monitor cash flow data (Appendix II: Cash Flow), thereby paying better attention to details about inventory. Inventory sitting on shelves or stored in boxes is essential capital that was not performing for the business. 1Carmen should seek out an accounting or business class at a local small business center, perhaps a community college, or engage the help of someone who could provide assistance with basic accounting ski lls. Having access to capital was promising but Carmen, by evidence, was somewhat vulnerable, lacking basic knowledge, skills and abilities to adequately utilize and â€Å"make effective use of financial capital† (Datar, 2009). Posterior Reversible Encephalopathy (PRES) Posterior Reversible Encephalopathy (PRES): A Rare Presenting Feature of Pheochromocytoma Abstract Hypertension in young is mostly due to secondary causes and one of them is pheochromocytoma. These are catecholamine secreting enterochromaffin tumors causing paroxysmal hypertension. Adrenal pheochromocytoma presenting as posterior reversible encephalopathy syndrome (PRES) is very rare and has not been described in literature so far. Here, we report a case of previously healthy adolescent boy, who presented with acute onset severe headache, blurring of vision, generalized tonic clonic motor seizure and altered sensorium. His blood pressure was 234/126 mm Hg. The magnetic resonance imaging (MRI) of brain showed hyperintense signal on T2-weighted and fluid attenuated inversion recovery (FLAIR) images in bilateral parietal and occipital regions. High blood pressure and classical MRI findings were consistent with the diagnosis of PRES. Abdominal ultrasonography (USG) revealed a right adrenal mass. A diagnosis of pheochromocytoma was confirmed by abdominal triple phase contrast- computed tomography (CT) and 24-hour urinary metanephrine assay. After the blood pressure was stabilized with alpha and beta blockade, adrenal tumor was surgically excised. Histopathologic examination of tissue confirmed the diagnosis of pheochromocytoma. The MRI brain showed complete resolution of hyperintense signals (T2-weighted and FLAIR images) on two-months follow-up. He was symptom free at six months and one year follow-up. Pheochromocytomas are rare catecholamine secreting enterochromaffin tumors. The patients of pheochromocytoma usually present with spells of headache, sweating and palpitations due to excessive catecholamines. The cerebral manifestation of pheochromocytoma are uncommon. Posterior reversible encephalopathy syndrome (PRES) as a presenting feature of pheochromocytoma is rare. Case Report A 14-year-old previously healthy boy presented in emergency department with acute onset severe holocranial headache, blurring of vision, generalized tonic clonic motor seizure followed by altered sensorium. There was no previous history of nausea, vomiting, diarrhea, flushing, excessive sweating, migraine, autoimmune and connective tissue disorders, drug abuse, toxin exposure, hypertension or diabetes mellitus. He had no similar illness in past and family history was negative. His pulse rate and blood pressure were 130 beats per minute and 234/126 mm Hg, respectively. He was confused but followed simple commands. The pupillary size and light reaction were normal on both sides. Fundus examination showed bilateral papilledema. He was moving all the four limbs equally without asymmetry on painful stimulus. Plantars were bilaterally extensor. Signs of meningeal irritation (neck rigidity and Kernigs signs) were negative. Complete hemogram, serum electrolyte, renal function tests and other biochemistry including thyroid function tests were normal. Serum anti-nuclear antibodies (ANA), anti-double-stranded DNA antibody (anti-dsDNA) and ELISA test for human immunodeficiency virus (HIV) were negative. X-ray chest was normal. Electrocardiography (ECG) showed tachycardia. Magnetic resonance imaging (MRI) of brain showed hyperintense signal changes on T2-weighted and fluid attenuated inversion recovery (FLAIR) images in bilateral occipito-parietal regions. No restriction was seen on diffusion-weighted images (DWI) [Figure 1]. CT angiography of brain vessels was normal. High blood pressure and classical MRI findings were consistent with the diagnosis of PRES. On further evaluation, abdominal ultrasonography (USG) showed right adrenal mass. An abdominal triple phase contrast-enhanced CT scan revealed heterogeneous, contrast enhancing adrenal gland mass lesion measuring 32 x 26 mm suggestive of pheochromocytoma [Figure 2]. The diagnosis was confirmed by 24-hour urinary metanephrine assay. The plasma aldosterone concentration (PAC) and plasma renin activity (PRA) ratio (PAC/PRA) was 10.4. The 24- hour urinary normetanephrine was 31,572 Â µg/24 hour (normal: 63-402 Â µg/24 hour), urinary metanephrine was 1,524 Â µg/24 hour (normal: 32-167 Â µg/24 hour) and plasma noradrenaline level was 18,635 pg/mL (normal: 0-400 pg/mL). Patient was managed intensively with nitroprusside infusion to reduce blood pressure. Injectable phenytoin was administered according to body weight to control seizures. Once patient was stabilized, he was started on oral alpha-blocker prazosin (20 mg/day) followed by beta-blocker propranolol (40 mg/day). After adequate alpha and beta blockade, patient was planned for surgery and resection of adrenal mass was done. Histopathologic examination confirmed the diagnosis of pheochromocytoma without invasion of the adrenal capsule [Figure-3]. The MRI brain showed complete resolution of hyperintense signals (T2-weighted and FLAIR images) and 24-hour urine metanephrines were normal on two-months follow-up. He was asymptomatic at six months and one year follow-up. Discussion Pheochromocytomas are neuroendocrine tumors arising from chromaffin cells of the adrenal medulla or extra-adrenal paraganglia. The classical features like holocranial headache, palpitations, hypertension, hyperhidrosis, hyperglycemia and hypermetabolism are due to excessive catecholamines production in these tumors. The cardiac complications such as arrhythmias, myocardial infarction and sudden deaths are associated with cardiotoxic effects of high blood catecholamines levels. Pheochromocytoma is a rare cause of secondary hypertension and accounts for 0.5% to 2.0% of all causes of hypertension in children. The neurological complications (ischemic or hemorrhagic stroke) are related to increased platelet aggregation, hypertension and vasospasm due to high catecholamines levels. The various potential triggers of pheochromocytoma crisis leading to hemodynamic instability are stress, blood loss, surgery and anesthesia. The diagnosis of pheochromocytoma is confirmed by 24-hour urinary meta nephrine and normetanephrine levels. Clinical features of reversible posterior leukoencephalopathy syndrome (PRES) are acute onset headaches, vision loss, seizures and altered sensorium. It is mostly due to hypertension, however other common causes are chronic renal disease, uremic encephalopathy, ergot alkaloids, steroids, chemotherapy, vasculitis and tumors. Pheochromocytoma is one of the rare cause of PRES due to secondary hypertension. However, to the best of our knowledge, acute, life-threatening initial presentation of pheochromocytoma as PRES has not been described in literature. Magnetic resonance images (MRI) of brain typically shows hyperintense signal changes on T2-weighted and fluid attenuated inversion recovery (FLAIR) images due to vasogenic edema. It mostly involves the cortical and subcortical white matter of the parietooccipital, frontal and temporal regions. Cerebral hemorrhage, cytotoxic edema and contrast enhancement are atypical imaging findings of PRES. The parietooccipital cortex and subcortical w hite matter were affected in our case. Classically, these signal abnormalities are reversible on antihypertensive therapy. The pathophysiology of PRES is still poorly understood, however, various hypothesis have been proposed. Severe hypertension causes deranged autoregulation of sympathetically mediated cerebral arterioles. It leads to increased permeability in the blood-brain barrier and causes vasogenic edema. Sympathetic innervation of the vertebrobasilar system is not as extensive or as complete as that of the anterior circulation. Therefore, PRES predominantly affects parietooccipital cortex and subcortical white matter. Other possible mechanism may be endothelial dysfunction due to circulating toxins or chemotherapy agents. There may be cerebral infarction or hemorrhage due to compromise of the microcirculation by pressure from surrounding vasogenic edema. The exact etiology of the seizure remains unknown, but may result from effects of the pheochromocytoma on reducing seizure threshold via its actions on metabolic or hypertensive parameters. Our patient had high levels of circulating catecholamin es, produced autonomously by tumor. Once vasogenic edema subsided with antihypertensive therapy, all the abnormal MRI findings vanished. Clinical topography (acute onset headache, visual blurring, seizure and altered sensorium), high blood pressure and typical MRI findings suggested diagnosis of PRES. In our patient, other differential diagnosis such as thrombocytopenic thrombotic purpura (TTP), hemolytic uremic syndrome (HUS), encephalitis, systemic lupus erythematosus (SLE), brain mass lesions and drug toxicity were ruled out by appropriate clinical and laboratory investigations. In our patient, hypertension was detected for the first time on hospital admission and before that he never had any symptoms like headache, palpitations, perspiration or diarrhea. Our patient is of clinical interest as pheochromocytoma presented with life-threatening hypertensive encephalopathy. Management of pheochromocytoma requires aggressive approach including fluid resuscitation and antihypertensive therapy (both alpha and beta blockade) followed by surgical resection of tumor. Conclusion Pheochromocytomas are catecholamine secreting enterochromaffin tumors causing paroxysmal hypertension. Our patient is of clinical interest as an acute, life-threatening hypertensive encephalopathy (PRES) as a presenting feature of adrenal pheochromocytoma has not been described in literature so far. Pheochromocytoma should be ruled out in every young patient with acute hypertensive encephalopathy. Figure Legends Figure 1.
